Looks like it barfed becase you don't have the Qt.Labs.Shaders module installed. Try the latest git revision; I've commented that part out because it wasn't doing anything anyway.
Actually, it's not necessary to credit the site unless the author specifically requests it in the description of the download (of course, site credits are always appreciated). ;)
Sorry to throw the book at you on this, but site policy is that we need you to upload the actual content, and not just previews (I would have mentioned this earlier, but I didn't realize that the model files weren't included).
If you'd be willing to upload them, I'll do my best to implement a download counting mechanism on OGA, since I suspect other people would be interested too. If not, I'll have to de-publish this submission, as it doesn't contain the actual files.
As for the license, you can choose whichever of the listed that licenses you prefer, including CC-BY. Just link to opengameart.org somewhere in your credits.
Use your mouse to select menu items then click on the target. The cure spell currently freezes combat, so don't use it.

It's confirmed by the authors at this point that this will never happen, unfortunately.

All you need to do is credit the author.
Greets!

No need to ask. CC-BY and CC-BY-SA allow for commercial use. :)
Just include my name in the credits (and release any modifications to the sound as CC-BY-SA, per the requirements of the license).
Yes, absolutely. :)

Glad to hear it!
Best of luck with your project, and let me know where I can check it out when you're done. :)

Yes, absolutely, as long as you give credit per the description. :)
What sort of game are you making, out of curiosity?
Hey, just thought I'd chime in here. If BVH files are large uncompressed, I'd rather people just zip them and sumbit that. :)
If need be, I can explicitly allow the format, but it seems fairly uncommon.
